Just seen in Don Bush's "Friendly Editor" column (pp. 38-39):
Technical writers' greatest gifts to users are elements
outside the text: headings, running heads, and indexes.
The best gift of all is a good index. Unfortunately,
[many] publishers seem to prefer an easy, software-
generated table of contents... Indexes are far more
useful, but they do require more skill to create. They
need to address subjects that users actually seek, not
just frequently used words...
