I use rev histories in specs, not user docs, though I've done so in the past
at other companies. Changes to content (errata, addendums, etc.) get
assigned incrementally to the right of the decimal. The only thing that can
change the number to the left of the decimal is changes to the product. That
can sometimes be a fine line and should involve discussions with
development/engineering because one person's feature enhancement for Product
A can be someone else's new Product B.
