You don't need Photoshop to create your own icons. Microsoft Paint will
work just as well.
Create your own .bmp file of the correct dimensions (16x16, 48x48, or
whatever size), and of the color depth you need (16 bpp, 24 bpp) and save it
as an .ico. Actually, all you really need to do is go to Windows Explorer,
and change the extension of the desired file from .bmp to .ico.
Alternatively, you can use one of many programs designed specifically for
creating icons (and even animated cursors).
I have successfully used Microangelo98 (don't know if it has a more recent
version), but there are more to choose from, including some free ones.

I'd like to apply a custom icon to a little utility I've created, but I need
to create a graphic file with an .ICO extension (an icon file).
Does anyone know how to do that? I have Photoshop, which I've used to create
the graphic. But it doesn't allow me to just save it as an .ICO.
