When I started I was documenting software from the source so, although it was fairly stable, you were still playing catch-up but almost in sysc!
Then I worked for a defence company where hand-over of the docs was anything up 6 months after the product was handed over - so we had two releases, first draft so they could test it etc and then final release incorporating any feedback
My last job was a mix of the standard - documenting prototypes and playing catch-up AND, occasionally, documenting finished products. Done this once or twice where the product was aimed at a specific release date and we actually had time to complete the documentation before it was released!
And now I never see what I document as it doesn't really exist! Basically we design virtual components so there's nothing physically there - great reliance on the SMEs at the moment though I now know what they're talking about most of the time!
