Heres my workaround - is this what you mean? This works for me. The trick is
step 2 & 4, if the doc is loaded whilst a printer has the custom page size
defined, word gets totally confused about what the page size should be (it
uses an offset index into its std paper list as it found the def - but it
wont match natch) and refuses to layout the doc in print format.

1) Setup PPD with Distiller's latest PS printer driver with page def.

2) Ensure Another printer is set as default.

3) Load doc with page settings customised.

4) Change default printer to customised ps from 1)

5) Invoke PDFWriter, use default printer.
