Document opens in Design Mode only

Greeting Whirlers,
I've inherited a document (form actually) that has a few fields embedded
within [drop-down box, input boxes, check boxes, radio buttons, etc]
The problem is, it only opens in design mode.  I've saved it after I was out
of design mode - no luck; I've password protected it - no luck; I've read M$
"help" - no luck.  I'm hoping that someone out there, more adept than I,
could offer some assistance.  Please respond directly, I'm on digest and I'm
a little against the wall on this one, time wise.
