Excel's online Help has helpful procedures. In the search, enter "headers"
(w/out quotes). Here's what I found:
_Create custom headers and footers_
You can have only one custom header and one custom footer on each worksheet.
If you create a new custom header or footer, it replaces any existing custom
header or footer on the worksheet.
Click the worksheet.
On the View menu, click Header and Footer.
To base a custom header or footer on an existing built-in header or footer,
click the header or footer in the Header box or Footer box.
Click Custom Header or Custom Footer.
Click in the Left section, Center section, or Right section box, and then
click the buttons to insert the header or footer information - such as the
page number - that you want in that section.
For Help on an option, click the question mark and then click the option.
To enter additional text for the header or footer, enter the text in the
Left section, Center section, or Right section box.
To start a new line in one of the section boxes, press ENTER.
To delete a section of a header or footer, select the section that you want
to delete in the section box, and then press BACKSPACE.
Note Make sure the header or footer margin allows enough space for the
custom header or footer. More information about setting the header and
footer margins.
Tips
To include a single ampersand (&) within the text of a header or footer, use
two ampersands. For example, to include "Subcontractors & Services" in a
header, type Subcontractors && Services.
To start numbering pages with a number other than 1, use &[Page]+ or
&[Page]- followed by the number you want to add or subtract from the actual
page number. For example, to print starting with number 3 on the first page,
type &[Page]+2. To print page 4 with number 3, type &[Page]-1.
You cannot insert graphics or cell references in a header or footer. Use
print titles if you want to repeat cell contents or a graphic on every
printed page. How to use print titles.
