According to Adobe, the workaround described in an earlier message (setting up the custom page size in the Print dialog box) DOES NOT WORK in NT 4.0 or its derivatives, including Windows 2000.
That's a drag, because the workaround (which was a hassle, but worked with PDFMaker, too) was the only way to get a custom page size. I had hoped they would solve the problem in Acrobat 5.0, but our experiments show that is not the case.
As a result, we have to maintain a single Windows 98 machine on which we can create PDFs in the custom page size.
