You can use AuthorIT to produce HTML 3.2 and XHTML 1.0 (in addition to several
other formats).

AuthorIT's output includes the navigation pane on the left for the TOC, Index,
and optional Search tabs.

You use Dreamweaver for the HTML files and Deva for the navigation (TOC, Index,
and Search). DW is an HTML editor, but can be used as an authoring environment
(much like other tools that let you edit one topic at a time).
