I've never seen the writer credited in external documents but it's not
that uncommon for internal documents - as somebody said, it makes it
easier if there are any questions. What I have seen done (and do myself
here) is to include a Change History table at the front of the document
which lists
For internal docs these are clearly visible. For external docs these are
formatted as hidden/white text so that it's not visible. So if there
are comments on User Guide X, v1.2, we can go and check who produced
v1.2 etc.
