That's exactly what I do. I write internal documentation that is kept on the intranet site. The index page lists contact information for the developers responsible for each application. Periodically, I will bring a printout of the index to their staff meeting and have the team review it for updating. This format was already in place before I got here, and having the users be able to contact the developers was what the developers wanted (or at least their management wanted it).

I see no problem with putting his/her name in the doc. It is an internal
doc after all.
In fact, you should include his/her e-mail address and phone number so that
people with questions, comments, or complaints can contacte him/her.
