Dick Margulis recommended using Acrobat Catalog and Acrobat Reader with
Search. This option provides some of the features we're looking for, but
it would only be usable online and I did want to provide something
printable to the users.
Susan Gallagher and Roib LaMora recommended altering the headings I wanted
to include in the references to include some form of identifier, include
the identifier in the index reference, and globally replace the identifier.
That sounds like a very workable solution, and that's probably the
direction I'll try first.
