Dian, you should eventually get the TOC links working. However AFAIK PDFMaker never looks at the index. I believe I read somewhere that there are third-party tools to handle the index links, however.
To your basic problem: Adobe has a KB article that may help you. It explains what you need to uninstall, what files you need to delete manually, and what menu selections you need to make when you do the reinstall. I don't have it in front of me, but if I dig it out later, I'll post the link.
