Conclusion and summary.
I installed Word 2000 along side Word 2002, re-installing Acrobat
afterwords. PDF Maker now makes a perfectly book-marked pdf on the big old
nasty file, as long as it is opened in Word 2000. The TOC links and
internal cross-references are also fine. This solution means that I have
to start a Word version first and then open the file, but that is easy
enough with short-cuts on the start-bar. Just for the record, the
combination of Word 2002 works fine with Acrobat 5.05 as long as the
source file is small enough - but I don't want to have to experiment with
file sizes or let the programs dictate the size of a book!
Thanks to everyone who helped, and especially to Martin Polley who wrote
to me off-list with the solution I ended up using.
