It depends on how the responsibility is assumed. It's a safe thing to say
where I am, since we're not the kind of people that use it as an opportunity
to see the speaker as a target of blame. Rather, we treat it as a learning
experience.
Further, if a manager or an officer of the company says it, those of us in
the trenches can breathe a sign of relief, knowing that we did the best we
could under the circumstances.
Besides, I'm in Canada. I've seen people apologize to parking meters :>
