Regardless of where you put the feedback request, there's a mathematical
relationship between the number of readers and the amount of feedback
you receive. In my experience, I'd say you might get one comment for
every thousand readers. In short, definitely include the feedback link,
but don't expect much from it.
We include an email address in the preface material of our printed
manuals, on every page of our help system (as part of the footer) and at
the end of all of our Flash tutorials. This generates a lot of spam, but
not much genuine feedback. Mind you, if your documentation is being read
by half a million people, the number of comments should increase
appropriately. Thanks. DB.

I am interested in what people think about requesting feedback from
readers for a user guide.
My feeling is that I should provide people with an e-mail address for
any comments or suggestions, but I'm not sure if this is the norm, or
whether it is generally felt to be a good / bad idea.
Also, if this is something that other writers have included in
documentation, where have you placed it? Proudly at the front? Or tucked
away at the back in the hope that nobody actually finds it?!
Any thoughts on this would be greatly appreciated.
