If writing skills are so valuable (ie non-common, non-trivial), why doesn't the
marketplace then pay writers the same as doctors or programmers?

Sure I can, but is your statement correct to begin with? For example, can you show me a programmer who is paid as much for programming as J.K.Rowling is paid for writing?
