Word template headers and footers

Hi,

I am wsing word, trying to create a template that has headers and footers
that run even/odd pages and are the same throughout the document. How do I
create the template to  use the headers and footers I want? 

How do I create the template to start numbering on the first chapter, and
not insert section breaks throughout the document.
