Just change the questionnaire to new questions.
Make up a madlibs-like thing that they can fill out with nouns, adverbs, names of other people in the company, etc. (This would have to change per person to work, though).
Do something like an "email roast" circulating through all the coworkers except the birthday person, then present to the birthday person.
