Paul,

RD fields don't actually put the content of the "reference document" into the container doc, as you should see by paging through the the container doc.  (There's nothing there but a TOC, RD fields, and an Index, right?  So that's what you're going to get in a PDF or any other printout of that file.)

Try converting each of your RD fields to an INCLUDEFILE field, without updating the TOC or Index; then create the PDF.
