Hi Jennifer, welcome to the list.

So the bottom line is, you want to do version control on some files, you're doing it manually, and human error is doing its stuff.

Two solutions come to mind:
a) don't do it manually, look into automatic document repositories. These are like libraries where you check in and check out documents, and the system keeps track of all the changes for you. It's essential to software development. You could ask in the IT Dept. what they use.
b) if you must do it manually use a more intuitive numbering system ...
TibbsJS_rev_00.doc could be
29_sept_03_TibbsJS.doc
You'd probably still have human error though.

The best way to get rid of human error is to get rid of the humans and let a machine do what it's best at...
