Are these chapters separate FM files or one big file?
Are you using <$chapnum>? How is your book numbering
set up?

Are your TOC/xref markers for your headings misplaced?

Ctrl+Alt+Click on a TOC entry and see where it goes.
The heading location is irrelevant in proofing this...
you need to locate the markers.

Is FM 7.0 installed with the latest updates from Adobe.com?
