My thinking on the matter is that Glossaries should NOT be indexed. A
Glossary is a thing for quick-reference, like an Index, but it's
different from an Index.
Now I've seen some indexes that have glossary items and refer the
reader to the glossary page, but I don't think that's necessary (Just
'see Glossary' is good enough). But I think that glossary items do not
have to be in the index.
