You probably you wouldn't ever say any of the four. You would say, "Add a formalpara template to the XSL Stylesheet to handle the recent dtd changes." Here "formalpara template" refers to a bit of XSL code that describes what to do with the element, formalpara.

Two other terms to get a grip on:

XSL Processor: An application that uses XML files as input and XSL Stylesheets as instructions to "transorm" the XML source into something else.

XML Parser: An application (possibly built into your XSL Processor) that parses and validates the XML before passing it to the XSL Processor. Don't forget (you will anyway) that like character entities like & n b s p ; are never seen by XSL. They are resolved by the parser.
