The short answer to your second question is no, Word provides no native
method for creating a linked index. One commercial solution is Steve
Hudson's Indexing tool (see http://www.geocities.com/word_heretic/Tools.html), which creates a linked
index in Word. I believe that it also transfers as a linked index when
distilled to PDF (to answer your first question). But it isn't cheap!
I have no commercial interest in Steve's products and have not tried them,
but he has a solid reputation in the Word experts' community.
