In your first example, I agree that "hazardous or unidentified substance
containers" would be the way to go. However, in the second example, if
someone can choose "either license information or registration information,
or both," then that's the best way to put it without using "and/or," unless
you're still talking about how to craft a heading, in which case I think
"license and registration information" is indeed the best choice.
