They're formatting marks, like the pilcrow for paragraph ends or the
middle-dots that indicate spaces. As I recall, a square dot before an
element means it bears a style that may affect page layout, e.g.
"Keep with next".
Since headings are born as "Keep with next", they get the square dots.
