Not trying to be negative here, but I tend to follow the line of
thinking that when someone asks how you are doing, it is a rhetorical
question. I don't think most people really care, they are just being
cordial.
Now, there is a segment of people who do geniunely care, but I believe
for the most part the question is an extension of 'Hello'.
