I think your suggestion of Incident Resolution Summary might be a good
one. Alternatively using your description I might call it a "System
Outage Report." I agree that "Post-Mortem" is inappropriate--it makes
it sound like either your customer or your service is deceased...
At the moment I have no suggestions for templates.
