Strongly agree. Hyperlinks in headers are easily missed by many readers,
and emphasizing/linking the first use in a passage is consistent with
the design of most printed textbooks. If I wanted to link from the
header as well, I'd use a glossary or (i)nfo icon following the term,
similar to the way Wikipedia uses an icon to indicate external links.
