Sometimes there is little or nothing you can do. You can try to talk to
somebody in authority and explain your concerns. But if they don't want to
take any notice, then you have little further means to influence the
situation.
At that point, you have some tough questions to ask yourself, such as:
1. Is the stress of seeing work done badly likely to affect your health?
(Especially if the disorganisation results in you having to work long hours
against impossible deadlines to get late features into the documentation.)
2. Is the company organising its development so badly that it is likely to
go under as a result of the bad reputation it is acquiring in the market?
If the answer to either of these questions is "yes", then you may be
well-advised to jump ship as soon as practicable and start looking for
another job in a better-managed organisation. But be aware that almost all
software companies suffer from what you have described to a letter or
greater extent, so a job with a perfectly-run organisation is going to be
pretty much impossible to obtain.
