I tend to avoid markers in headings, as the content should have the
marker while the heading is in the TOC. Beyond that, the marker is
either directly before the indexed word or at the start of the
paragraph being tagged. The latter is more convenient for
localization but less convenient because in-line image markers go
there and distinguishing between the two is bothersome.
