I'm abysmally ignorant of forms and form fields, so I'm not much use to you there. (Maybe the ASK field?)
However, for any template, in the VBE you can open Microsoft Word Objects > This Document object, select Document from the Object list, then select New or Open from the procedure list and write code that you want executed whenever a new document is created from the template, or whenever the document is opened (provided it can find the template, which contains the code).
