I once worked with someone who was asked to submit a letter of resignation because of performance issues. A few months down the road, he called to tell me he was applying for a tech writing job (not his duties here), and he asked if I would act as a reference.
It was a very uncomfortable situation (thankfully, it was a phone call), but I had to tell him that I would not feel comfortable acting as a reference and I told him why. I then went on to tell him that I *did* think he did an excellent job when training customers (because, he was a good teacher).
You think this was uncomfortable but I think you were
pretty lucky: I once got a call from a company asking
for a reference for a writing manager with whom I'd
worked as the editor on her project, and she hadn't asked
me whether she could use my name! I actually didn't think
that she was a very good manager but I was able to
escape by telling the company that contacted me quite
truthfully that I had not worked directly for her and
therefore didn't feel comfortable commenting on her
as a potential employee. I then immediately called her
and told her the same thing, adding that if she was
going to use someone as a reference, the least she
could do was ask first.
