Have you tried HTMLDoc? I think there's a version for Windows. I've used
this on Linux to do a quick and dirty web-to-PDF conversion that yielded a
fairly decent fake book format. You get a TOC, but no index. It works best
if the HTML pages are formatted in a way that makes it easy for the program
to pull out the TOC, but I bet Doxygen output would fit that bill.
Let's try it...
Well, maybe not. The TOC it came up with for Rosegarden was pretty lame:

1.3 GUI
1.4 Sequencer
You might fare better with a better organized Doxyfile though.
