Anybody have a 1099 contract they will share?

Hiya, folks.

I got laid off last week, and now, lo and behold, the co. realized that
there are a few loose ends they need help with.  They want me to contract
with them for a bit. I could use a simple 1099 contract, as I can't seem to
find mine.

Any help would be appreciated!
