The last time I looked into this (2006), I read good things about
www.pdf-file.com, which now costs $30. Never tried it myself...

Subject: Conversion of PDF to Word
What is the best way to convert a PDF file into a Word file?
And I mean a good conversion--not just converting the PDF's text into
Word, but getting the Word document to resemble the original PDF closely
in every aspect: images, formatting, etc.
In Adobe Acrobat Professional, I tried doing "File... Save As... Word."
The resulting Word doc leave much to be desired. Its appearance is
certainly not identical to the original PDF...
