Back in 2000 or so, Peter Schorer wrote a small book called "How to Create Zero-Search-Time Computer Documentation" (ISBN 1573980110); a PDF of the book can be downloaded at no cost from http://www.zsthelp.com/book/ .
He has a strong opinion on the right way to do indexing; you may find it interesting.
