Naming Conventions for Documents

Hello 
 
My boss sent this to us today, It is called 'Naming Conventions for Electronic Documents."
 
Some of this I have in my head, and some in the documents I have saved over the years, but nothing that captures all of it. Do any of you have documents of a similar nature covering this topic?
