Yes, I have a detailed log of what changes I make to the doc. I note the HTML file, corresponding PDF file, the project that caused the change, what I did with the version number, the date I worked on it and the specific changes I made, including paragraphs or entire topics that were completely new. This doc is for my own use and not shared with any of my co-workers. I do not use a source management system. I do run a DOS batch file on Friday afternoon so I can always go back to that if I have to.
