I second the endorsement for WorkRave. No more eyestrain or tension
headaches.

Now that I'm on a Mac, I use MacBreakZ (
http://www.publicspace.net/MacBreakZ/). Not open source like WorkRave, but
similar in functionality, and still a good investment of $25.
