Very interesting.
On the subject of index markers, I long ago adopted a convention of
putting all index entries for a paragraph in a single marker placed
before the first word of the paragraph. The size of the marker is
rarely anywhere near the 255-character limit. For that case, you could
use multiple markers. A single index entry should not be that big.
