I recently had a similar problem, and discovered that it makes a difference whether the caption is applied to the picture or the frame. If the caption is INSIDE the frame, Word can't see it. I went through and made sure that all of my images either did not have a frame, or that the caption was applied to the frame, and after that the TOC worked fine.

Subject: Word 2007 - List of Figures Issue
I'm having problems inserting a correct List of Figures in a TOC.
I'm using Word 2007, Windows XP.
For each graphic in the document, I've used the Insert Caption tool to insert a numbered caption (for example: Figure 1. Pressure Gauge). But when I generate the List of Figures, a few of the figures don't appear in the list--and it's the same few each time. I can't figure it out. I've deleted and re-generated the List of Figures; I've deleted and re-entered the captions; I've deleted and re-inserted the graphics themselves. I've checked the styles (Caption style for captions). What else am I missing? Any help appreciated!
