Further to this, and taking a hint from your remark, I printed out the book using PDF Creator - and it came out fine, with all the bookmarks functional.

Are you using "Save as PDF?" There were a lot of problem reports on
this in FM9 and from what I've been reading so far it has not been
fixed in X.
