In Acrobat 9 Pro (Win 7), I open the usual Print dialog, and at the bottom
of the dialog box (to the left of the OK button), there's a Summarize
Comments button. When I click that, the Summarize Options dialog box has a
checkbox to say whether to include Pages containing no comments, and also
gives you several layout options.
