Both of those popped in my mind and I quickly dismissed them for being, er, ancient pedantry? "Data entry form," is almost as bad, but it seems to be the best option if calling a form a "form" violates style rules when there is no other common term for a form other than "form." Less informative choices of places to put data entry include widget, page, area, tab, screen, and "where the data goes."
