In Acrobat Pro, there are several Migrate, Export, and Import Comments
options. I've used them with success, but for managing comments for PDFs
that started out the same. So I'm not sure what would happen if the new PDF
had different content, which is what it sounds like you'll have?
The other likely gotcha is if you're using Acrobat X Standard instead of X
Pro. Although shared reviews are supposed to work for Reader viewers
regardless of whether you're using Standard or
Pro,(http://www.adobe.com/products/acrobat/product-comparison.html), as the
many threads on this forum show, it does not unless you do some extra work:

upgraded from Standard to Pro.
