What if you first create a protected Word form, and then convert that document to PDF? Word forms use the same elements as PDF forms: text boxes, check boxes, etc.^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^
