I am only vaguely familiar with AutoHotKey. Are you talking about scripting Mac apps? I assume, the accessibility feature requires direct user interaction (i.e., press tab multiple times until the right item is selected).
macOSâ built-in scripting engine, on the other hand, is Automator. I have very little experience with it and understand that it is a little outdated and neglected by Apple. There should be plenty of support material available on Automator by Apple that can tell you whether it is feasible for what you want to do.
