The latest (17th) edition says "The name of a living person should,
wherever possible, correspond to that personâs preferred usage."
I believe that's about people who make a point of using nonstandard
capitalization or punctuation, e.g. bell hooks. Is there anyone who
insists on no period after their middle initial? Even Jennifer 8. Lee
uses a period now.
Famous science-fiction fan Forry Ackerman's preference was Forrest J Ackerman, or occasionally Forrest J (no period) Ackerman. Other fans generally acceded to his request, biographies and style guides less so.
Then there is the poet e e cummings.
